Birth Certificates












General Information


We are excited to announce our recent partnership with the Florida Department of Health and Bureau of Vital Statistics. Certified copies of birth certificates dating back to 1930, from all counties within the State of Florida may now be obtained at our Main and Sebastian Offices. Since birth records are required for obtaining or renewing Florida driver’s licenses and State Identification cards, the partnership between the Tax Collectors Office of Indian River County and the Florida Department of Health makes this process easier by consolidating services and allowing residents to complete both transactions at the same office and substantially improve our service to the public. The fee for the first certificate is $18.25 and $10.00 for each additional copy. Requirements to obtain a birth certificate are outlined below. No phone or mail application requests are permitted.


Florida Birth Certificate Requirements:


Applicant must provide valid picture identification. Acceptable forms include:

Driver license or ID Card


Military ID


Applicant must be eligible to receive the birth certificate by meeting one of the following:

Be the child named on the Florida birth record AND 18 years of age or older

Be the parent of the child named on the Florida birth record


Applicant must be prepared to provide full information listed on the birth record:

Full name of child at birth

Date of birth

City or county of birth

Mother's full name including maiden surname

Father's full name (when listed on birth record)


Applicant must be prepared to provide full information as requester:

Full name

Relationship to child named on birth record

Mailing address

Daytime phone number

Application must be completed in person.
